Experimental Drug Elraglusib Shows Promise in Improving Survival for Pancreatic Cancer Patients

An experimental drug called elraglusib nearly doubled one-year survival rates for patients with metastatic pancreatic cancer in a randomized Phase 2 clinical trial, offering a rare bit of good news for one of the deadliest common cancers.

The trial: Actuate-1801

The trial, known as Actuate-1801, enrolled 233 patients with previously untreated metastatic pancreatic cancer across 60 sites in North America and Europe. Participants who received elraglusib alongside standard chemotherapy (gemcitabine and nab-paclitaxel) had a median overall survival of 10.1 months, compared with 7.2 months for those who received chemotherapy alone. The results were published in Nature Medicine.

Doubling the one-year survival milestone

The most striking result was at the one-year mark: about 44 percent of patients on the elraglusib combination were alive at one year, compared with about 22 percent of those on chemotherapy alone — effectively doubling one-year survival. Overall, the risk of death was reduced by approximately 37 percent among patients receiving elraglusib.

Why pancreatic cancer is so hard to treat

Pancreatic cancer remains one of the most lethal common cancers, often diagnosed at advanced stages when surgery is no longer an option. Standard treatments like FOLFIRINOX and gemcitabine-based chemotherapy regimens have offered incremental gains, but survival rates overall remain low, which is why a result like elraglusib’s stands out.

Other approaches in the pipeline

Elraglusib is one of several novel therapies in clinical trials aiming to improve pancreatic cancer outcomes. Other research is exploring chemotherapy combined with immune checkpoint inhibitors for patients with genetic mutations such as BRCA1 or BRCA2, as well as high-dose and precision radiotherapy techniques designed to shrink tumors enough to make surgery possible in previously inoperable cases. Researchers are also combining kinase inhibitors and stroma-modifying agents with immunotherapy to help treatments reach cancer cells through the tumor’s dense protective environment.
